Please note - The term "accommodations" is used throughout to include not only disability-related accommodations, but also disability-related services and/or auxiliary aids.

Requesting Your Accommodation Letter

Students who have previously received an Accommodation Letter will need to request a new Accommodation Letter for each quarter.  Please select the name of your Disability Adviser or Learning Specialist to complete the Accommodation Letter online request form:
